This comic is suffering from discontinuity issues. Even though the subject is completely fantastic we still like to know the normal rules of physics still operate for the mundane things depicted therein.

two things:
1. in the first frame and in the recent strips i’ve checked, the shackle is on the right arm. in the 2nd and
3rd panel it’s on the left.
2. gravehouse’s should wound has disappeared.

In the second panel, is Gravehouse just being dramatically shaded or does he have a bit of the inner-demon thing going on too?

You are correct on both points. The artist is now spending a period of time in the monster gulag.
Hopefully this will sharpen his focus.

Poor Merlin’s spent too much time drawing dragons instead of remembering which arm the plot device is on. Tut tut.
